Required Documents

 

All families applying for financial aid are required to submit the documents below to Westminster School before a financial award can be offered.

  • Westminster School Financial Aid Information Form
  • Copy of Parents’ Financial Statement (PFS) filed with SSS (http://www.sss.nais.org/). This form should only be completed after your child has been admitted to Westminster School.
  • 2009 tax return (Form 1040 with all supporting schedules)
  • (New applicants are required to submit two years of tax returns with initial application.)
  • 2009 Form W-2’s
  • 2009 Form 1099’s
  • Business tax information
  • 2009 Student tax return (if applicable)
  • Two current pay stubs from each wage earner
  • IRS Form 4506T
  • Business Farm Statement (if applicable)
